Please be advised that any overseas immunisation records need to be taken to the Immunisation Clinic to be translated onto an MOH approved certificate. If you have already done this and given us a copy at school, thank you.
All other immunisation records need to be fully reported on an immunisation card approved by the MOH. These immunisations are to include all those done from birth to 5 years of age. (Complete) A copy is to be held by the school for inspection when the immunisation teams do their checks this year. If you have already done this and given us a copy at school – thank you.
We understand this process can be frustrating as there are often large queues currently at the hospital, however this is now a requirement.
Failing to comply with this could result in a fine for the parents and the school. Students will no
longer be permitted to attend school without full immunisation records
